Repair Authorization

What this means?

Before we work on your device, we ask every customer to review and sign our Repair Authorization Form in store. This document outlines how repairs are performed, what to expect during service, and how we handle things like diagnostics, pricing, data, warranties, and communication.

Electronics repairs involve working with complex, sometimes damaged hardware and software. In some cases, issues may already exist, parts may be fragile, or full testing may not be possible before service begins. This authorization helps set clear expectations and protects both you and Tekrehab by explaining these risks upfront.

It also gives us permission to:

  • Diagnose and repair your device as requested

  • Communicate with you about your repair, including by text message

  • Install necessary software or updates related to the service

  • Handle unclaimed devices after the required pickup period

Our goal is transparency, fairness, and good communication. If you ever have questions about the repair process or this authorization, we are happy to walk through it with you before you sign. Below is the form you will sign in store prior to us performing any service.

TEKREHAB REPAIR AUTHORIZATION

THIS AGREEMENT IS MADE BY AND BETWEEN TEKREHAB LLC, DOING BUSINESS AS TEKREHAB, AND THE “CUSTOMER”. BEFORE SIGNING THIS AGREEMENT, THE CUSTOMER HAS HAD THE OPPORTUNITY TO READ THE “TERMS AND CONDITIONS” FOR SERVICES RENDERED BY TEKREHAB.

_____________I UNDERSTAND THAT *IF TEKREHAB IS UNABLE TO PERFORM A PRE-CHECKLIST FOR SERVICE, THAT THEY DO NOT ASSUME RESPONSIBILITY FOR ANY ADDITIONAL FUNCTIONS WHICH MAY NOT BE WORKING PROPERLY AFTER THE REPAIR. ANY FUNCTIONS NOT WORKING PROPERLY AFTER THE REPAIR ON DEVICES THAT TEKREHAB CANNOT PERFORM A PRE-CHECKLIST ON ARE NOT COVERED UNDER THIS REPAIR AND WILL ONLY BE FIXED AT THE EXPENSE OF THE CUSTOMER.

TERMS AND CONDITIONS

1. DISCLAIMER

I understand that {Tekrehab} is not an authorized service dealer for Apple, Samsung, HTC, LG, or any other manufacturer, and that repairs rendered by {Tekrehab} may void manufacturer warranties for this device. {Tekrehab} does not assume any liability or warranty in the event that the manufacturer warranties are voided.

“Device” includes phones, computers, tablets or any other electronic device or part brought in to Tekrehab LLC for repair.

Tekrehab will only perform and provide computer services, repairs, and upgrades as requested by the customer.

Tekrehab will conduct honest, reasonable, and considerate services. The goal is to provide the highest quality of service and support, but specific results cannot be guaranteed.

PC/Device service/repairs are provided as a service. There may be circumstances under which your computer or device cannot be repaired. It will have to be rebuilt or upgraded. (Examples: Age of PC or Device, repair/replacement parts obsolete (memory chips, motherboards, etc.)

I fully understand that in the process of working on your computer/device equipment, that there is a poten al for data loss.

I, the owner of this PC/Device, have backed up all informa on on the hard disk in which I want to preserve. I also understand that {Tekrehab} is not responsible for any lost data, and I hereby waive any legal ac on against this company. I understand that the data on this device is not the responsibility of {Tekrehab} and may be lost during the repair process without my prior knowledge.

The length of me required to service/repair your computer cannot be predicted.

You authorize the technician(s) providing the service or repair to install an-virus and any other necessary.

Our Technicians will not browse through your hard drive or memory cards looking at your data; however, a technician may inadvertently see data during the course of performing necessary work. Please remove any personal or private files you do not want others to see.

I understand that as a result of submitting my PC/Device for repair that it could be subject to data loss.

I understand that {Tekrehab} is not responsible for cellular devices not claimed within thirty (30) calendar days and computer devices for sixty (60) calendar days from the date of completion. I also understand that {Tekrehab} will recycle or sell any device not picked up a er the deadlines stated above from the date of completion regardless if item was repaired or not.

If the PC/Device is left after Sixty (60) days and Thirty (30) days, respectively, it will be considered abandoned and will become the property of {Tekrehab}.

2. PRICE AND PAYMENT

In order to provide you with an accurate price for your repair/service, your device will need to be diagnosed. You understand that if diagnos c equipment is required to diagnose your PC/Device, or the PC/Device casing, frame or any part(s) is opened or disassembled as a requirement to diagnose your PC/Device, there will be a fee assessed in the amount of $15.00. If the repair is performed by Tekrehab, we will apply the $15.00 fee toward the price of the repair. “Device” includes phones, computers, tablets or any other electronic device or part brought in to Tekrehab LLC for repair.

No PC/Device will not be released until the diagnosis fee and/or total repair cost (if repaired), has been paid in full.

Any quote for repair is not a final billing price, and that additional billing my be necessary, and is subject to change.

3. SUPPORT, LIABILITY, & WARRANTY

Customer satisfaction is our utmost importance, and we will conduct ourselves in a professional, reasonable and timely manner. Also, taking into consideration on the circumstances and nature of the technical problems.

In the case of accidental damage of data to your system or data loss caused by already existing problems in your system such as viruses, bad configured software, or hardware problems/failures -You agree to hold Tekrehab and any person(s) associated with Tekrehab or involved in the work being performed for you harmless from damages resulting from such problems.

4. TEXT MESSAGE COMMUNICATION CONSENT

1. By signing this Repair Authoriza on, I consent to receive SMS/text messages from Tekrehab, LLC at the mobile number I have provided.

These messages may include, but are not limited to:

• Repair status updates

• Authorization requests •

Pickup notifications

• Invoices, receipts, and payment reminders

• Follow-up communications related to my service

2. I understand that message frequency may vary and that standard message and data rates may apply.

3. I understand that consent to receive text messages is not a condi on of purchase or service, and that I may opt out at any me by replying STOP to any text message or by notifying Tekrehab directly.

4. Tekrehab does not sell or share mobile numbers with third par es for marketing purposes.

BY SIGNING, I UNDERSTAND THAT {TEKREHAB} OFFERS NO VERBAL OR WRITTEN WARRANTY, EITHER EXPRESSED OR IMPLIED, REGARDING THE SUCCESS OF ANY REPAIR. I HEREBY EXPRESSLY WAIVE ANY AND ALL CLAIMS AGAINST TEKREHAB FOR ANY DAMAGES TO THIS DEVICE OR DATA THAT IS LOST DUE TO THE REPAIR EFFORTS RENDERED BY TEKREHAB. I UNDERSTAND THAT {TEKREHAB} OFFERS 90 DAY WARRANTY ON BATTERY REPAIRS; 90 DAYS PARTS & LABOR, 90 DAY WARRANTY ON SCREENS AND OTHER INTERNAL REPAIRS: 90 DAYS PARTS & LABOR. BY SIGNING BELOW, THIS CONFIRMS THAT I HAVE READ, UNDERSTAND, AND AGREE TO BE BOUND BY THE TERMS & CONDITIONS SET FORTH IN THIS DOCUMENT.
